The answer to all of these questions (and more!) is A Fantasy Dynasty.

I have had a gander and it looks quite similar to what I am trying to achieve, but I feel like creating a new story to start from scratch, the original author hasn't been on in a few years and hopefully, there won't be too many similarities, and I am not really going for a fantasy setting...

Start with the setting and the worldbuilding. Be sure and put some social inequality and some disease in there, along with archaic ideas about how to address these issues. Then just as the reader is settling in for some nice dynastic intrigue... (character limit)
