Okay, from a top-level you can either start out with the main character already a sissy, or they're going to become a sissy. You'll need some sort of challenge or conflict for the character to overcome. They might face discrimination, or they might have a particular attraction to some individual, or they need to make some money to afford a body modification they really want. If you want more porn and less plot, for example, you might have a sissy boi who wants to seduce the rich homophobic son of the chief executive of the company they both work for, so that they can get the money for breast implants. Well the easy route from there is having the MC fail the crossdressing contest. During the event the MC could however catch the eye or one of the, judges, contestants, spectators, or participants. The one who takes notice of the MC could be male or female, they approach the MC with the promise to either help them, win the next contest or other small ones in the area offering smaller amount of winner prizes. Or they could be wealthy and be looking to make a more sexual offer for the MC's time. Both paths end up with the MC being under the influence of a more controlling and dominating partner who shapes the MC into their preferred look. Another path the MC could follow is if after they lost the contest, feeling the weight of the debt on them they rush out of the contest building still cross-dressed and get approached by someone who finds them crying. Depending on how dark your story is this encounter could go very differently, from being convinced to going to a hotel willingly to getting gang-raped in a alley and everything in-between.
